Spring Hill Rehabilitation & Nursing Center, a state-of-the-art skilled nursing facility, invites you to join our dedicated team and deliver compassionate, skilled nursing care to our residents.

As an LPN, you will administer medications, perform assessments, support care plans, and collaborate with an interdisciplinary team to ensure safe, effective transitions and outcomes. A $3000 sign-on bonus is offered along with a comprehensive benefits package.

Qualifications

• Current LPN licensure in Pennsylvania.
• Willingness to learn and grow with the team.
• Ability to work FULL_TIME shifts, including days, evenings, and weekends.

Responsibilities

• Provide high-quality, patient-centered care to residents, including administering medications, changing wound dressings, and performing vital signs and treatments.
• Develop and implement individualized care plans in collaboration with our interdisciplinary team, ensuring that each patient's unique needs are met.
• Conduct routine assessments, monitor patient progress, and report changes to the healthcare team.
• Maintain accurate and detailed records of patient care, treatments, and outcomes.
• Collaborate with our rehabilitation team to facilitate smooth transitions and ensure continuity of care.
