Job summary

Upstate Caring Partners in Rome, NY is seeking a Licensed Practical Nurse for a 12-month school-based position. This full-time role focuses on daily nursing care for school-age/preschool children, documentation, and collaboration with the agency’s treatment team.

The LPN must hold NYS licensure and registration, have a high school diploma, and be able to assist children. A valid NYS driver’s license is required. Compensation is $25 per hour with competitive benefits.

Qualifications

Responsibilities

• Ensure documentation is accurate, contemporaneous, and regulatory compliant.
• Promote safety and well-being of children using procedures and equipment.
• Attend to personal and program needs that children cannot do alone.
• Keep nursing and admin teams informed of medical/dietary concerns.
• Provide support and peer consultation among participants.
• Encourage student input in program activities.
• Oversee safety and upkeep of equipment and supplies.
• Participate in treatment team activities, meetings, trainings, and committees.

The Licensed Practical Nurse (LPN) School Age/Preschool position is responsible for providing daily nursing care to all school age and/or preschool children, participate in Agency activities and complete required documentation.

Core Responsibilities

• Ensure that documentation is accurate, contemporaneous, and effectively communicates all information required to be regulatory compliant.
• Promote and oversee the safety and well-being of the children through use of written, verbal procedures, safety equipment and awareness of any potential dangers.
• Attend to personal, self-care and other program needs that the children cannot independently accomplish.
• Keep the nursing and administrative team advised of medical and dietary concerns.
• Provide support and consultation and initiate peer support among program participants.
• Promote student input in the development of program activities.
• Oversee the safety and well-being of the children.
• Oversee and upkeep the maintenance of equipment and supplies.
• Participate in activities as part of the treatment team that may include meetings, trainings, and committees.

Qualifications

• High School Diploma required
• Valid NYS Licensure as a Licensed Practical Nurse and current NYS Registration Certificate.
• Ability to lift and/or assist children in your care.
• Must have a valid NYS Driver’s License
