Job summary

Upstate Caring Partners is seeking a Licensed Practical Nurse to provide daily nursing care, manage medications, and update the RN/Nurse Practitioners. The role supports adults and youths, with training provided and Monday–Friday daytime hours.

The LPN will implement Individual Program Plans, assist participants with self-care, and work with a multidisciplinary team to ensure safety and high-quality care. Travel is required within Central New York; NY licensure is mandatory.

Qualifications

• Valid NYS LPN license and current NYS registration.
• A.A.S. degree or high school + specialized trade certificate.
• Strong interpersonal and communication skills; travel required.
• Basic computer skills (Windows, Outlook, Adobe Acrobat, Word, Excel).

Responsibilities

• Provide daily nursing care and assist with medications under RN supervision.
• Train and orient AMAPs to program procedures.
• Schedule appointments and participate in agency activities.
• Maintain required documentation and provide updates to RNs/Nurse Practitioners.
• Oversee safety and well-being of participants and supply maintenance.
• Collaborate with the treatment team to support participant goals.

The Licensed Practical Nurse (LPN) is responsible to provide daily nursing care, order medications, provide updates to the RN and Nurse Practitioners, train and orient AMAP's, schedule appointments, participate in agency activities and complete required documentation.

Core Responsibilities

• Promote the individuals’ independence by implementing the Individual Program Plan.
• Attend to personal, self-care and other program participant needs.
• Provide support and consultation and initiate peer support among program participants.
• Promote program participant input in the development of program activities.
• Oversee the safety and well being of the program participants.
• Oversee and upkeep the maintenance of equipment and supplies.
• Participate in activities as part of the treatment team.
• Ensure compliance with all pertinent government and agency regulations and operating standards.

Qualifications

• A.A.S. Degree or High School plus Specialized Trade Certificate.
• Valid NYS Licensure as a Licensed Practical Nurse and current NYS Registration Certificate.
• Strong interpersonal and communication skills.
• Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel)
• Travel is required. Must have a valid NYS Driver’s License.
• Ability to attain and maintain SCIP-R and CPR certification.
