Job summary

BAYADA Home Health Care’s Phoenix Skilled Office is seeking a compassionate Licensed Practical Nurse (LPN) to join our private duty nursing team serving pediatric and geriatric clients in the Phoenix area. You will provide one-on-one home care, follow physicians' orders, and document care activities with accuracy and empathy.

Qualifications

• Graduation from an accredited LPN program with Arizona license.
• Current LPN license in good standing in Arizona.
• At least one year of LPN experience in private duty/home health care.
• Trach experience preferred but not required.

Responsibilities

• Travel to one client’s home and provide private duty nursing care.
• Follow physician orders and document assessments, interventions, and evaluations.
• Perform ongoing assessments and monitor clients’ conditions, reporting changes.
• Deliver care with autonomy while maintaining client safety and comfort.

Our Phoenix Skilled Office is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Skilled Nursing team in either pediatric and geriatric private duty roles. We believe our clients and their families deserve the highest quality care- delivered with compassion, excellence, and reliability.

Compensation and Schedule:

• Overnight and Day Shiftsof 8-12 hours available
• Starting pay: $30-35 per hour (client-specific)

Qualifications for a private duty LPN:

• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
• A current nursing license in good standing in the state of Arizona
• One year of experience working as a Licensed Practical Nurse
• Trach experience preferred, but not required
