We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality nursing care to adult and geriatric residents in a supportive care environment.
The LPN will work as part of the nursing team to provide direct resident care, monitor resident needs and changes in condition, support safety and well-being, and ensure care is delivered according to established plans, policies, and applicable standards.

Provide direct nursing care to residents in accordance with individualized care plans, policies, procedures, and the LPN scope of practice.
Monitor residents' physical, emotional, and psychosocial status and promptly report changes to the appropriate nursing supervisor.
Assist residents with activities of daily living, including hygiene, grooming, dressing, toileting, elimination, incontinence care, positioning, and skin care as appropriate.
Promote a safe, clean, and comfortable resident environment.
Respond promptly to call lights and alarms and follow established fall-prevention and safety procedures.
Monitor the appropriate use and condition of medical and safety equipment and report malfunctions as needed.
Support residents' nutritional needs, including meal assistance, feeding, nourishment, and use of adaptive equipment when required.
Encourage and support rehabilitative and restorative activities such as range-of-motion exercises, ambulation, transfers, positioning, and proper use of assistive devices.
Respect and communicate resident preferences, choices, rights, refusals, and concerns.
Document care and resident status accurately and in a timely manner.
Communicate effectively with residents, families, caregivers, and members of the interdisciplinary care team.
Follow infection-control procedures and applicable requirements related to occupational exposure to bloodborne pathogens.
Maintain a professional, compassionate, and resident-centered approach at all times.
Current LPN license in the applicable state.
BLS certification through the American Heart Association (AHA) required.
High school diploma or GED required.
Ability to provide appropriate care to adult and geriatric residents.
Strong communication, organizational, and interpersonal skills.
Ability to recognize and report changes in resident condition.
Ability to work effectively as part of a nursing and interdisciplinary team.
$30.24-$32.58 per hour
Starting rate of approximately $30.24/hour for new graduates, with compensation increasing based on relevant experience
