Job summary

Woodbury Heights Nursing and Rehabilitation Center in Town of Oyster Bay, NY, is seeking an LPN to join our compassionate, patient-centered care team. You will provide bedside nursing, administer medications, monitor residents, and help implement personalized care plans in collaboration with RNs and physicians.

This role requires availability every other weekend and shifts 7am–3pm, 3pm–11pm or 11pm–7am. NY LPN license plus CPR/First Aid and at least 1 year in long-term care are required.

Qualifications

• Must have NY LPN license and CPR/First Aid certification.
• Minimum 1 year of experience in long-term care.

Responsibilities

• Provide compassionate, patient-centered care to residents.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document vital signs, symptoms, and changes in resident conditions.
• Assist in developing and implementing personalized care plans.
• Collaborate with RNs, physicians, and other healthcare professionals to ensure high-quality care.
• Respond to medical emergencies and provide timely support.
• Maintain accurate and up-to-date medical records.
• Promote a positive, supportive environment for residents and staff.

Schedules:

• MUST BE ALBE TO WORK A MINIMUM OF EVERY OTHER WEEKEND
• 7am to 3pm, 3pm to 11pm or 11pm to 7am
• shifts on weekdays available as well.

• Provide compassionate, patient-centered care to residents, helping them live comfortably and with dignity.
• Administer medications and treatments as prescribed by physicians.
• Monitor and document vital signs, symptoms, and changes in resident conditions.
• Assist in developing and implementing personalized care plans.
• Collaborate with RNs, physicians, and other healthcare professionals to ensure the highest standard of care.
• Respond to medical emergencies and provide immediate support.
• Maintain accurate and up-to-date medical records.
• Promote a positive, supportive environment for both residents and team members.

Requirement:

• Valid LPN license in New York State
• Current CPR and First Aid certification
• 1 year of experience in long term care.
