Job summary

Northern Tier Counseling is seeking an LPN to join our outpatient psychiatry team in Towanda/Mansfield clinics. You will collect health data, manage medications under psychiatric directions, and coordinate care with Providers to support clients’ wellness.

The role requires PA LPN licensure, CPR certification, and 1–2 years of psychiatric experience preferred. Strong communication and teamwork are essential for effective continuity of care.

Qualifications

• Graduate from an approved L.P.N. program.

The Licensed Practical Nurse position will function as a professional member of the clinical team. Under the direction of the Outpatient Program Manager, the LPN will provide identified nursing skills for the delivery of quality psychiatric care within an interdisciplinary and integrated clinical framework within the N. Towanda and the Mansfield Outpatient Clinic office as well as the Children’s Adolescent Day Treatment Program.

• Gather initial health and psychiatric information from clients and provide ongoing psychiatric management while following identified treatment plan for each client.
• Gather history and collect data such as reason for visit, health history, vital signs, and additional relevant information.
• Assist in gathering and organizing information for all clients on medication and complete case management services for the clients as needed. This will be done through completing prior authorizations for medications, communicating with pharmacy representatives and insurance companies.
• Responsible for making referrals to other agency’s for services that the Providers (MD, PA-C, CRNP) feel appropriate and medically necessary.
• Effectively perform the nursing responsibility for the administration of oral and intramuscular medications; will prepare and/or provide direction to the client relating to their oral medication trays. The LPN will strictly follow the psychiatric provider’s medication orders. The LPN will show effective and consistent review of written/verbal psychiatric provider orders to confirm accurate client medication record with corresponding written documentation for all actions done by the nursing staff. Under no circumstances should the LPN provide medication to a client that has not been ordered and documented by the Providers (MD, PA-C, CRNP).
• Consistently demonstrate the practice of effective interpersonal communication skills with coworkers and community professionals for the purpose of continuity of care within the medication management department. Each contact will be clearly documented with appropriate plan of action detailing and defining responses and effective resolution including follow up with the psychiatric Providers and Outpatient Program Manager as needed.
• Contact the identified pharmacies to obtain accurate information regarding client prescriptions, verification of Provider orders and relate changes and/or new orders in medication prescribed by the psychiatric Provider to pharmacy.
• Trained and maintain CPR certification and be prepared for medical and psychiatric emergencies; cardiac and respiratory emergencies, treatment of convulsive disorders, and related neurological conditions and diabetic management. LPN will demonstrate efficient and effective response in a calm and concerned manner with the client and family/support system.
• Knowledgeable of the Mental Health Procedures Act and be able to assist the psychiatric Providers in the process of voluntary or involuntary hospitalizations. LPN will possess an ability to interact with community/hospital professionals in the hospitalization process to ensure effective communication and transfer of written information for continuity of care.
• Interact as an integral team member in the medical department and will consistently work to facilitate communications between all staff and the psychiatric providers.
• Trained in providing nursing functions within the NTC Children/Adolescent Partial Hospitalization Program, at the level of recognized skill and training. The LPN will complete duties as outlined above in this Job Description while working with the Children/Adolescent Partial Hospitalization Program.
• Participate in program team meetings, and any other staff meeting on an as needed basis and as directed by Outpatient Program Manager or the Providers.
• Responsible for ongoing in-service training programs to maintain nursing licensing standards and will attend ongoing training to gain continuous knowledge of psychiatric management of mental health and addiction disorders and clinical interventions, with emphasis on psychopharmacological treatment.
• Participate in weekly or as needed clinical supervision with the psychiatric Providers and will participate in educational/training opportunities as directed by their clinical Director and the Providers.
• Proficient in their job position within six (6) months. If at any time the LPN feels they need more training/education, the LPN is to submit a request for such through their Outpatient Program Manager.

Requirements

Educational Requirements:

• Graduate from an approved L.P.N. program.
• Permit or L.P.N. License to practice in the state of PA.
• One to two years of inpatient or outpatient psychiatric experience preferred.
• Ability to interpret policy and resolve problems.
• Good interpersonal skills and ability to communicate effectively.
• Cardiopulmonary Resuscitation Training.
• An overall understanding of current psychiatric medications and side effects.
