CenterWell Senior Primary Care in Florida is seeking an LPN to provide clinical support under licensed clinician guidance. The role covers direct patient care, medical assisting duties, and patient communications in a busy clinic setting.

The LPN will perform intake, vital signs, medication reconciliation, and document health history while coordinating with clinicians to ensure high-quality care. A Florida license and CPR/BLS are required, with ongoing CEU obligations and adherence to policy.

Qualifications

• Active and unrestricted LPN license in Florida with current certification/registration.
• CPR/BLS Healthcare Provider Certification within 90 days of employment is required.
• Ongoing professional development and continuing education maintained.

Responsibilities

• Processes incoming and outgoing calls/messages in a professional manner.
• Performs patient intake and collects information to update records.
• Ensures clinician-reviewed consents are completed.
• Pre-encounter prep, including identifying care gaps.
• Supports clear communication with patients, families, visitors and colleagues.
• Gathers patient data via interview and observation.
• Assists clinician during treatments, examinations, and procedures.
• Prepares patients for examination, records vital signs and meds reconciliation.
• Assists with arranging lab and ancillary testing.
• Performs duties consistent with MA/MA-RN scope and sanitizes equipment.
• Manages inventory and maintains exam rooms.
• Participates in quality initiatives and clinical process improvements.
• Documents patient data and education per policy.
• Schedules appointments and coordinates testing/clinician consultations.
• Maintains compliance with policies and demonstrates task competency.

Role Responsibilities

• Processes incoming and outgoing calls/messages in a professional manner.
• Performs patient intake, obtains necessary patient information to file and update patient records.
• Ensure consents are reviewed and completed by clinician.
• Performs pre-encounter prep prior to patients visit, including identifying open care gaps.
• Supports and demonstrates effective and professional communication with patients, family members, visitors, and colleagues.
• Gathers patient data through interview and observation.
• Assists the clinician during medical treatments, examinations, and procedures.
• Prepares patients for examination, performs and documents vital signs, and records patients' health history and medication reconciliation. 9. Assists with arranging laboratory and/or other ancillary testing.
• Perform duties and skills consistent with a trained certified medical assistant/registered medical assistant/nurse.
• Prepares and sanitizes equipment, examination rooms and instruments.
• Manages inventory of medical supplies and equips exam rooms with appropriate supplies.
• Embraces and participates in quality initiatives, goal attainment, and collaborates on clinical process improvement activities.
• Provides routine treatments and administers approved medication per clinician order within scope.
• Documents patient data, treatments, procedures, and patient education per organizational policies.
• Assists the clinician by acting as a liaison on behalf of the patient to schedule appointments; provides patient with scheduling information to obtain testing, and/or clinician consultation.
• Maintains compliance with all policies and procedures.
• Demonstrates competency in specific tasks and skills as defined annually.
• Other duties as assigned.

Required Qualifications

• Active and unrestricted LPN license in the state of Florida. Certification, registration, or license must be current or must obtain initial certification, recertification, updated registration, or license with 90 days of employment.
• CPR/BLS Healthcare Provider Certification within 90 days of employment.
• Assumes responsibility for professional development and continuing education.

Preferred Qualifications

• IV Certification
• Experience working within a healthcare setting

Annual Requirements

• Must maintain current license, certification, or registration.
• Must complete required CEUs for licensure/certification
• Must maintain current BLS certification.
• EMR and clinical competencies completed and signed as assigned.

Additional Information

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state's driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
