Job summary

San Simeon by the Sound is seeking compassionate, skilled LPNs to join our nursing team. The LPN provides direct nursing care to residents, administers medications and treatments, and collaborates with the interdisciplinary team to ensure exceptional resident care and outcomes.

Responsibilities include administering meds, monitoring residents, documenting accurately, supervising CNAs, and participating in care planning. A NY LPN license in good standing and LTC experience are preferred.

Qualifications

• Current New York State LPN license in good standing.
• Long-term care, skilled nursing, or rehabilitation experience preferred.
• Strong clinical assessment and organizational skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.
• Reliable attendance and punctuality.

• Administer medications and treatments as prescribed by physicians.
• Monitor residents for changes in condition and promptly report findings to the RN Supervisor, Unit Manager, or physician as appropriate.
• Complete accurate and timely nursing documentation.
• Perform resident assessments within the scope of practice.
• Supervise and direct Certified Nursing Assistants (CNAs).
• Assist with admissions, discharges, and transfers.
• Maintain infection control and safety standards.
• Participate in care planning and interdisciplinary team meetings.
• Ensure compliance with facility policies, state regulations, and nursing standards of practice.
• Provide compassionate, resident-centered care while maintaining resident dignity and respect.
