Job summary

Oak View Health and Rehabilitation in South Carolina is seeking a dedicated LPN to join our care team. You will monitor patients, document changes, and collaborate with physicians to devise individualized care plans that deliver high-quality resident care.

Qualifications

• LPN license required.
• Prior experience preferred but new grads welcomed.

Responsibilities

• Monitor patient's condition and assess their needs to provide the best possible care.
• Observe and interpret patient's symptoms and change of conditions, and communicate them to physicians.
• Collaborate with physicians and other nurses to devise individualized care plans for patients.
• Perform routine procedures (blood pressure measurements, administering injections etc.) and fill in patient's charts.
• Adjust and administer patient's medication and provide treatments according to physician's orders.
