Job summary

MJHS Home Care is seeking a Licensed Practical Nurse to provide advanced clinical home care services, including patient assessment, medication management, and family education, under RN supervision. You will conduct revisits to homebound patients, implement the plan of care, and document visits within 24–48 hours.

Responsibilities include observing and recording clinical data, reconciling medications, and performing nursing treatments as directed by the plan of care.

Qualifications

• LPN license required (NY State).
• Minimum of one year Med/surg experience in a hospital or long-term care facility.
• Good computer skills required.

Responsibilities

• Observe, measure, record and report clinical data related to a patient’s health status.
• Collect, communicate and document signs or symptoms indicating a decline in a patient’s health status.
• Reconcile and administer prescribed medications according to the physician’s plan of care.
• Perform nursing treatments (eg. urinary catheterization, sterile dressing change, tracheal suctioning) as prescribed.

Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient’s recovery from illness, injury or surgery at home.You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.

Under the direction of a Coordinator of Care RN, the Licensed Practical Nurse will perform revisits to patients in theirhomebased on a skilled need. You will implement the plan of care and evaluate the patient's treatment. You will document the visit electronically within 24-48 hours.

• Under the direction of an RN, accurately observes, measures, records and reports clinical datarelated to a patient’s health status
• Accurately collect, communicate and document any sign(s) or symptom(s) outside of establishedparameters indicating a decline in an individual patient’s health status
• Under the direction of an RN, accurately reconciles and administers prescribed medicationsaccording to the physician prescribed plan of care
• Under the direction of an RN, accurately performs all nursing treatments (eg. Urinary catherization,sterile dressing change, tracheal suctioning) indicated in the physician prescribed plan of care
