MJHS Home Care is seeking a Licensed Practical Nurse to provide advanced clinical home care services, including patient assessment, medication management, and family education, under RN supervision. You will conduct revisits to homebound patients, implement the plan of care, and document visits within 24–48 hours.
Responsibilities include observing and recording clinical data, reconciling medications, and performing nursing treatments as directed by the plan of care.

Under the direction of a Coordinator of Care RN, the Licensed Practical Nurse will perform revisits to patients in theirhomebased on a skilled need. You will implement the plan of care and evaluate the patient's treatment. You will document the visit electronically within 24-48 hours.
