Job summary

MJHS Home Care is seeking a Licensed Practical Nurse to provide advanced home care services, including patient assessment, medication management, and patient/family education, under RN supervision. You will complete visits and document electronically within 24-48 hours.

Requirements include NY State LPN license, 1 year Med/surg experience, and a valid driver's license with reliable car; strong computer skills are also needed.

Qualifications

• NY State LPN License required.
• Minimum 1 year Med/surg experience in a hospital or Long-term Care facility.
• Must have a driver's license and car.
• Good computer skills required.

Responsibilities

• Observe, measure, and report clinical data related to a patient’s health status.
• Collect, communicate, and document signs or symptoms outside established parameters indicating decline.
• Reconcile and administer prescribed medications under RN supervision.
• Perform nursing treatments per physician's plan of care (e.g., catheterization, sterile dressing changes, tracheal suctioning).

Someone with your depth of compassion, clinical insight and strong decision-making capabilities is the ideal person to map out and oversee the process of a patient's recovery from illness, injury or surgery at home. You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.

Under the direction of a Coordinator of Care RN, the Licensed Practical Nurse will perform revisits to patients in theirhome based on a skilled need. You will implement the plan of care and evaluate the patient's treatment. You will document the visit electronically within 24-48 hours.

• Under the direction of an RN, accurately observes, measures, records and reports clinical datarelated to a patient's health status
• Accurately collect, communicate and document any sign(s) or symptom(s) outside of establishedparameters indicating a decline in an individual patient's health status
• Under the direction of an RN, accurately reconciles and administers prescribed medicationsaccording to the physician prescribed plan of care
• Under the direction of an RN, accurately performs all nursing treatments (eg. Urinary catherization,sterile dressing change, tracheal suctioning) indicated in the physician prescribed plan of care

• NY State LPN License required
• Minimum of one year Med/surg experience in a hospital or Longterm Care facility required
• Must have a drivers license and car
