Position Overview

SCOPE OF ROLE:

Coordinate all medical/psychiatric care and medications for residents ensuring proper documentation, follow‑up and regulatory compliance. Assist with hands‑on training of residential staff to ensure this compliance is ongoing and current. Secondary responsibilities include resident care and oversight in ADL's. The scope of the unique nursing practice uses the framework necessary to ensure safe and competent nursing care involving each person and their circle of support. ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Handles telephone calls and inquiries; provides necessary follow‑up; keeps family and management notified when necessary
  • Acts as liaison between Residence, Day Program, Physicians and families on all resident medical related issues
  • Assures that Manager/Coordinator and residence staff are fully informed and instructed about treatment/medication changes verbally, via log book, MAR, Med change board and ensures that instructions are being carried out by staff on next shift
  • Updates RN daily on person supported medical status
  • Review medication documentation from previous day - Notify RN of any errors detected
  • Review Vital Signs/Glucose readings from previous day and Bowel Movement charts, notify RN of any irregularities
  • Communicate pertinent medical information to RN, Manager, Day Programs and families as needed
  • Write communication log notes that pertain to medical visits or any medical situations and update medical summaries
  • Notify RN and Manager of medication or treatment changes and plan for follow‑up
  • Report resident’s medical symptoms to the RN and follows instructions from RN
  • Notify RN, Manager and Behavior specialist of psychotropic med changes and in conjunction with RN obtain necessary consents
  • Responsible to implement all directives issued by RN and Manager as needed
  • Oversee coordination of all medication administration including verifying prescriptions, ordering medications, regulatory documentation, and staff administration of medications in conjunction with RN and under supervision of RN
  • Maintain medication inventory including prescription medications, over‑the‑counter medication and required first aid supplies are in stock at all times and that check expiration dates are routinely checked
  • Transports and accompanies residents on scheduled medical appointments and completes all necessary follow‑up upon return from appointment
  • Ensures maintenance of all medically related documentation (MAR, MD Consults, etc.)
  • Responsible to complete all documentation prior to end of shift and report all issues/concerns to RN and Residence Manager
  • Completes the documentation of Occurrence Reports as required
  • Completes necessary residential habilitation notes, progress notes and other required documentation
  • Reviews and makes daily entries into House Log Book on all pertinent resident/residence related issues
  • Monitors resident progress and notifies applicable others of concerns, changes in behavior or of progress
  • Knows and applies emergency procedures in event of a crisis including fire safety
  • Provides individual residents instruction and/or assistance with daily living skills and rehabilitation or social skills in accordance with Individual Service Plan
  • Supervises residents’ intake of medication as prescribed in accordance with OMRDD approved Medication Administration Procedure
  • Administer medications as needed
  • Review Precision Care at the beginning of each shift and follow up on all medical/pharmacy/nursing related entries
  • Enter new meds or med changes on MAR, complete resident specific information and retain a current Rx for all meds
  • Provides ongoing training to residents and staff in areas of need identified in Individual Service Plan in conjunction with RN & Manager
  • Provides hands‑on client specific training to staff members as directed by RN
  • Attends various meetings/in‑services, participates in staff meetings in conjunction with mandatory annual in‑services as directed by Residence Manager I RN

Licence Practical Nurse Registered Nurse Salary: Minimum - $43,000.00, Maximum - $46,000.00 Annually

REQUIRED EDUCATION AND EXPERIENCE

  • New York State License Nurse
  • Two+ years LPN experience
  • Minimum of One year experience in community health care services
  • Excellent written and verbal communication
  • Knowledge of Microsoft Office tools
  • Ability to provide crisis intervention utilizing good interpersonal skills with a high level of maturity and responsibilities

PREFERRED QUALIFICATIONS & SKILLS

  • Specialized training in I/DD
  • Basic knowledge of human services
  • Bilingual (Spanish/English) speaking preferred
