Hamilton Park Nursing and Rehab is seeking a Licensed Practical Nurse (LPN) to join our skilled nursing facility in Brooklyn, NY. Eligible direct hires can receive a $5,000 sign-on bonus.

As part of The Allure Group, you will provide direct patient care, observe residents, update care plans, administer medications, and coordinate with staff to ensure high-quality nursing services. The role requires NYS LPN licensing and vaccination, with opportunities for professional development.

Responsibilities

• Make rounds to observe residents and nursing care.
• Review and integrate Nursing Care Plan with medical orders.
• Provide transcription and execution of physical orders.
• Utilize staff capabilities to provide quality resident care.
• Order medications and supplies; maintain medication counts.
• Maintain accurate clinical records and documentation.
• Collaborate with inter-departmental staff to maintain nursing services.
• Meet with nursing personnel to identify improvements.
• Prepare to perform any LPN duties within scope of practice.
• Provide direct patient care when needed.

Licensed Practical Nurse (LPN) Responsibilities

• Make rounds to observe residents and nursing care
• Review and integrate the Nursing Care Plan to reflect physical emotional, restorative, rehabilitative, teaching, and goals with the medical care prescribed
• Provide proper transcription and execution of physical orders
• Utilize the capabilities of staff members to provide quality resident care
• Order medication and supplies for the nursing unit and residents
• Control the counting and recording of medications
• Maintain proper and accurate documentation and maintenance of clinical records
• Cooperate and communicate with inter-departmental and facility personnel to nursing services are being adequately maintained
• Meet with other nursing personnel to assist in identifying and correcting problem areas, and/or improvement of services
• Remain prepared to do any services or assignments within the scope of practice of an LPN
• Provide direct patient care when needed

Licensed Practical Nurse (LPN) Requirements

• Valid NYS LPN license
• Exceptional communication, customer service, and interpersonal skills
• COVID-19 Vaccination

Preferred

• Previous experience in long-term care
• Previous experience with Point Click Care
