Job summary

The Carlyle House, Inc. is seeking a Licensed Practical Nurse (LPN) to deliver safe, therapeutic resident care from admission through discharge in collaboration with physicians, families and the care team in Kettering, OH.

Responsibilities include measuring vital signs, assisting with personal hygiene, injections, wound care and accurate documentation, while ensuring a safe, clean environment and adherence to facility policies and OSHA rules.

Qualifications

• Licensed Practical Nurse (LPN) with valid license.
• Experience delivering patient care in a healthcare facility.
• Ability to lift up to 50 pounds and manage a flexible schedule.

Responsibilities

Job description

The Licensed Practical Nurse (LPN) is accountable for the delivery of safe and therapeutic resident care from admission through discharge in collaboration with the physician, family and other members of the multi-disciplinary health care team. Delivery of quality nursing care is assured through utilization of the nursing process. The goal of resident care shall be toward restoring and maintaining each resident’s capabilities at their maximum mental and physical level. Serves as a positive clinical role model for staff as well as a resource person. Supports the philosophy, mission and core values of The Carlyle House.

Physical Requirements:

This position demands good physical and mental health. The nurse aid will be required to lift, catty, walk, sit, push, pull and work a flexible schedule, must be able to move continuously during work hours and able to lift and/or carry 50 pounds.

Duties:

• Provides basic bedside care
• Maintains comfortable, orderly, safe and clean environment for patients
• Measures & records patients’ vital signs (such as height, weight, temperature, blood pressure, pulse, respiration)
• Assists with bathing, dressing, and personal hygiene of patient Prepares and gives injections and enemas
• Records food/fluid input/output
• Dresses wounds
• Assists with bed positioning, standing and walking of patients
• Collects samples for testing
• Performs routine laboratory tests as directed by supervisor
• Records prescribed medications.
• Reports adverse reactions to medications or treatments in accordance with the policy regarding the administration of medications by a licensed practical nurse
• Assists in relevant health teaching for patient and family/significant other
• Records all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate forms Performs other position-related duties as assigned, depending on assignment setting
• Completes required orientation as directed by facility
• Follows facility and OSHA safety rules and procedures while on assignment
