Job summary

Storybrook Care and Rehabilitation is seeking compassionate LPN/LVN nurses for full-time and PRN shifts in a skilled nursing environment. Sign-on bonuses are available for qualified candidates. Shift options include 6PM-6AM, 6PM-2AM, and PRN days, with 12s or 8s schedules.

Qualifications

• Graduate of an approved school of licensed practical/vocational nursing.
• Current LPN/LVN license in good standing.
• 1+ years nursing experience.

Responsibilities

• Assist in directing daily nursing care and monitoring of nursing assistants.
• Follow procedures for narcotics counting and medication administration.
• Communicate observations with Nurse Manager or Director of Nursing.
• Document changes in patient conditions and notify appropriate staff.
• Participate in shift handoffs to ensure accurate patient information.
• Assist in admission, orientation, and observation of new residents, ensuring proper orders are followed.

Work Schedule: Full-time and PRN shifts

Pay Rate: $28.00 - $40.00 / Hour depending on experience and qualifications

JOB SUMMARY:

This position to ensure that quality care is delivered through monitoring of staff performance, physician contact, care planning and daily resident observation. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established State and Federal regulations for skilled nursing facilities.

JOB DUTIES:

• Assist routine daily direction and monitor the work of the nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks.

• Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.

• Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.

• Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.

• Participate in report at the beginning and end of shifts to assure proper information about residents is communicated.

• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

• Assist in the admission, orientation, and observation of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure.

REQUIREMENTS / EDUCATION / EXPERIENCE:

• Graduate of an approved school of licensed practical/vocational nursing

• Current license, in good standing, as an LPN/LVN in the State

• 1+ years nursing experience

• Devotion to standards and principles of professional nursing
