Job summary

Rivaya of Des Plaines is seeking dynamic and compassionate LPNs to enrich residents' lives with attentive, high-quality care in a welcoming Extended Care setting. Our team delivers individualized nursing support and collaborates with physicians, therapists, and staff to meet each resident’s needs.

As an LPN, you will supervise nursing assistants, document admissions and transfers, administer medications, and prioritize rounds in a fast-paced environment.

Qualifications

• Valid Nursing License as required by the state.
• Ability to work every other weekend.
• Compliance with the state’s Health Care Worker Background Check requirements.
• Must be able to relate positively and favorably to residents and visitors.
• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.

Responsibilities

• Direct the day-to-day activities of the nursing assistants.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Prepare and administer medications as ordered by a physician and perform other drug administration functions per established policies.
• Make daily rounds of the unit and prioritize tasks in a fast-paced environment.

LPN Requirements:
