Job description

Overview

The Licensed Practical Nurse (LPN) provides nursing services to individuals residing in Intermediate Care Facilities (ICFs). The LPN reports to a Registered Nurse and is responsible for working within the scope of their license to ensure the physical health needs of clients are met and maintained on an individualized basis in accordance with policies, procedures, and current federal, state, and local standards, guidelines, and regulations. The LPN coordinates health needs with members of the client’s team, provides necessary training to team members, and delegates nursing tasks to certified direct service providers.

Essential Functions
  • Act on direction of a licensed physician, licensed dentist, or registered nurse to administer controlled and non-controlled medications (oral, rectal, injection); maintain medication accountability records, equipment, and supplies; perform skilled nursing procedures (e.g., apply and change dressings, catheterize); assist nurse in medical procedures; request, secure, and follow up on medical information of residents.
  • Provide general nursing care to residents; maintain first‑aid supplies and provide first‑aid treatment as necessary; screen for communicable and nuisance diseases.
  • Observe and document conditions and responses to nursing care, treatment, and medications; report unusual developments to the supervisor.
  • Supervise dietary services, ensuring residents unable to feed themselves are properly fed and that special utensils are available; note and record issues related to eating and fluid intake; report unusual problems to the dietitian and physician; provide instruction and training to residents in personal health‑related matters (nutrition, grooming, hygiene, safety).
  • Prepare and maintain written records and reports as required; ensure compliance with all reporting standards; accept on‑call rotation.
  • Participate as a member of the interdisciplinary team, working with other professional staff to assess, develop, implement, evaluate, and modify Individual Habilitation Plans.
  • Provide consultation services to staff, residents, parents, and significant others regarding health and safety issues; promote consistency of programming outside the residential environment; operate county vehicles.
  • Participate in infection control, pharmacy, therapeutics, committee meetings and others as necessary.
  • Perform other related duties as assigned.
Minimum Qualifications
  • Must be at least 18 years of age.
  • Graduate of an approved school of practical nursing.
  • LPN license from State Board of Nursing.
  • Must be able to communicate in English, both spoken and written.
Licensure / Certification
  • Ability to obtain and maintain First Aid, CPR (adult, child, infant); delegated nursing (training provided if not already certified).
  • Valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles.
  • Proof of auto insurance.
Experience
  • One year of experience working as an LPN with intellectually or developmentally disabled individuals (preferred).
Special Skills and Requirements
  • Detail‑oriented with competent literacy, writing, documentation, communication, and interpersonal abilities.
  • Ability to use tools and equipment as required – including pushing/pulling wheelchairs, lifting consumers with hoist lifts.
  • Ability to travel in and out of apartments to perform medication passes.
Physical and Environmental Requirements

The physical activities for this position involve:

  • Balancing, stooping, pulling, pushing, standing, kneeling, reaching.
  • Lifting (up to 50 lbs) and carrying (up to 50 lbs).
  • Crouching, crawling.

The essential sensory and communicative activities include:

  • Feeling, seeing, speaking, hearing.

Exposure to environmental conditions includes:

  • Inside and outside environmental conditions.
  • Exposure to blood and bodily fluids.
  • Combative residents.
