Description

The purpose of this position is to oversee, provide, and coordinate nursing care in compliance with facility policies and procedures and to evaluate residents’ care needs and take appropriate action. The Licensed Practical Nurse will care for ill, injured, or convalescing residents in the facility. The LPN may work under the supervision of a Registered Nurse.

Supervisory Responsibility

None- may direct the work of CNAs, PCAs, and/or RAs on shift.

Budget Responsibility

None.

Essential Job Functions – Principal Accountabilities
  • Administers medications, noting times and amounts on residents’ charts.
  • Observes residents, charting and reporting changes in conditions such as adverse reactions to medication or treatment and takes necessary actions.
  • Answers residents’ calls and determines how to assist them.
  • Measures and records residents’ vital signs such as height, weight, temperature, blood pressure, pulse, and respiration.
  • Provides basic resident care or treatments such as taking temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, massage, or performing catheterizations.
  • Works as part of a health care team to evaluate resident needs, plans and modifies care, and implements interventions.
  • Evaluates nursing intervention outcomes, conferring with other health care team members.
  • Assembles and uses equipment such as catheters, or oxygen suppliers.

Requirements
Minimum Requirements

Education: Associates degree in Nursing, or equivalent is required.

Experience: Experience performing the essential functions of the position is desirable.

Certification/License: Ohio Licensed Practical Nurse (LPN) license in good standing is required. Candidates are required to attain and maintain CPR certification within 90 days after hire. State background check standards must be maintained as a condition of continued employment. Must complete first aid training required by rule 3701-16-06 of the Ohio Administrative Code, within 60 days of hire.

  • Move resident as necessary, stretch, bend, stoop, squat, twist, crouch, kneel, and balance to access all resident body parts, observe skin color, detect vital signs and equipment alarms/messages.
  • Operate computer, read, remain in a stationary position, converse with others.
  • Legibly complete forms; perform data entry of information. Remain in a stationary position for extended periods.
  • Converse with others, write legibly, read and comprehend written/verbal information. Communicate instructions or demonstrate tasks.
  • Lift and carry up to 35 pounds frequently; up to 75 pounds occasionally.
  • Stand and walk frequently, sit for extended periods; simple grasp; fine hand manipulation; near vision; far vision; hear conversation; speak.
