Job summary

Centerstone in Columbia, TN is seeking a team-oriented Licensed Practical Nurse to join our behavioral health team. This office-based role supports therapists, nurse practitioners, psychiatrists, and case managers in delivering high-quality patient care from 8 to 5, Monday through Friday.

The LPN will perform comprehensive psychiatric assessments, monitor health status, administer medications, provide education, and collaborate with the treatment team to ensure safe, effective care.

Qualifications

• Graduation from an accredited nursing program required.
• Six months to one year of psychiatric or equivalent mental health nursing preferred.

Responsibilities

• Performs comprehensive psychiatric/mental health assessments and evaluates mental status.
• Monitors health status including vital signs, weight, nutrition, and disease states.
• Administers and monitors medications as ordered; assess side effects and EPS as needed.
• Provides phone triage and patient education; assists with accessing medications and prior authorizations.
• Participates in laboratory testing and maintains CLIA-waived environment.
• Collaborates with the treatment team and external providers to deliver care.

Evaluates mental status and refers clients to other treatment providers when necessary. Assumes primary responsibility and accountability for ones nursing practices as defined in the state specific Code of Nursing Practice, as it pertains to a licensed practical nurse and functions under the supervision of an RN or physician.he Columbia clinic is seeking a team-oriented LPN to join a team of therapists, nurse practitioners, psychiatrists, case managers, and support staff to deliver care that saves lives. Mental health experiences preferred. At least two years' experience in the field as an LPN. This is an office-based job. Hours of Monday through Friday 8-5 and is not telehealth or is not a work from home position.ESSENTIAL DUTIES & RESPONSIBILITIESPerforms comprehensive psychiatric /mental health assessments, evaluates mental status and refers to other treatment providers when necessary.Monitors changes in health status, including but not limited to vital signs, weights, nutritional status and disease states.Administers and monitors medications as ordered. Monitors medication effectiveness, side effects and compliance. May perform assessments for any side effects/EPS.Provides phone triage and addresses medication issues with consumer when necessary. Provides proper education and guidance as necessary. Assists patients with accessing medications including obtaining prior authorizations and accessing prescription assistance programs.Provides healthcare education to clients and/or families or staff as assigned.Provides CLIA Waived laboratory testing as ordered. Maintains CLIA waived laboratory environment including Quality Control testing and documentation. Monitors received laboratory values and reports abnormal values as required. Phlebotomy may be required.Provides employee health services as directed, including annual influenza vaccination clinics and staff TB skin testing.Works in collaboration with other members of the treatment team and in collaboration with external health care providers as necessary.
