Job summary

South Shore Rehabilitation in Chicago seeks a dedicated LPN to provide compassionate, patient-centered care in a collaborative nursing team. You will oversee medication administration, assist with daily living activities, and support residents through their recovery and daily routines.

You will contribute to accurate charting, coordinate with nursing assistants, and uphold safety standards in a fast-paced skilled nursing environment.

Qualifications

• Valid Nursing License as required by the state.
• Ability to work every other weekend.
• Compliance with the state's health care worker background check requirements.
• Excellent communication and interpersonal skills.

Responsibilities

• Direct the day-to-day activities of the nursing assistants.
• Complete required record-keeping upon resident admission, discharge, and transfer; chart per policies.
• Prepare and administer medications as ordered by a physician.
• Make daily rounds and prioritize tasks in a fast-paced setting.

LPN Requirements:

• Valid Nursing License as required by the state.
• Ability to work every other weekend.

• Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.

As a skilled, compassionate LPN, you will:

• Complete required record-keeping upon resident admission, discharge, and transfer; chart and document per facility policy.
• Prepare and administer medications as ordered by a physician and perform other drug administration functions per established policies.
• Make daily rounds of the unit and prioritize tasks in a fast-paced environment.
