Job description

Overview

The Licensed Practical Nurse (LPN) provides supportive nursing services within Burlington United Methodist Family Services’ residential programs under the supervision and direction of the Director of Clinical Services, Registered Nurse, physician, or other authorized healthcare provider, consistent with West Virginia Board of Nursing standards, WV residential licensing requirements, agency policy, and applicable federal and state regulations.

Essential Duties and Responsibilities
  • Conduct monthly medication administration record reviews for completeness, legibility, transcription accuracy, and policy compliance; document observed discrepancies and report concerns requiring clinical judgment to the Director of Clinical Services, supervising RN, or prescribing provider.
  • Review physician medication orders for completeness, required signatures, transcription accuracy, and consistency with medication administration records, including monitoring for follow-up documentation related to psychotropic medications.
  • Provide guidance, training reinforcement, and monitoring to residential staff regarding medication administration procedures, documentation expectations, infection prevention practices, health-related protocols, and implementation of nursing directives consistent with West Virginia LPN scope-of-practice standards.
  • Reinforce agency medication administration procedures and report observed medication variances, documentation concerns, or potential compliance issues to nursing leadership in accordance with agency policy.
  • Communicate observations, medication-related concerns, appointment follow-up needs, and health status updates with residential supervisors, case managers, counselors, physicians, psychiatrists, pharmacy providers, and nursing leadership as appropriate.
  • Assist with coordination and follow-up of medical appointments, physician recommendations, medication changes, laboratory requests, and health-related documentation requirements.
  • Monitor medication storage areas, medication cabinet procedures, medication expiration dates, and first aid supplies for compliance with agency policy and licensing requirements.
  • Support infection prevention, OSHA compliance practices, universal precautions, and residential health and safety initiatives through observation, reinforcement, and staff education.
  • Provide or assist with in-service training related to medication administration practices, infection control, OSHA requirements, documentation expectations, and health-related procedures under the direction of nursing leadership.
  • Assist with preparation for licensing reviews, audits, accreditation activities, medication record reviews, and health record organization.
  • Monitor and report health and safety concerns observed within residential programs to appropriate supervisory or clinical personnel.
  • Maintain confidentiality of protected health information, personnel matters, and sensitive agency information in accordance with HIPAA, WV confidentiality laws, agency policy, and professional ethical standards.
  • Support and facilitate positive interactions with youth, families, staff, providers, and community stakeholders as evidenced by professionalism, teamwork, respect for others, and maintenance of professional boundaries.
  • Demonstrate effective verbal and written communication skills, including appropriate use of agency communication systems and timely documentation practices.
  • Support the mission, vision, Sanctuary commitments, and core values of Burlington United Methodist Family Services through compliance with agency policies, procedures, licensing standards, and ethical practices.
  • Perform additional duties within the scope and responsibility of the position as assigned by supervisory or clinical leadership.
Supervisory Responsibilities
  • This position does not independently supervise nursing personnel.
  • This position may assist with reinforcement of medication administration practices and health-related procedures with residential staff under the direction of nursing leadership.
  • Responsibilities involving AMAP oversight, delegation, competency validation, or clinical determination will occur only as permitted under West Virginia law, Board of Nursing standards, and agency policy.
Qualifications
Education and Licensure
  • Possesses and maintains a current, active West Virginia Licensed Practical Nurse license in good standing.
  • CPR certification required or obtained within agency-established timeframe.
  • Valid driver’s license required.
Experience
  • Residential treatment, behavioral health, child welfare, psychiatric, pediatric, or community health settings (preferred).
  • Experience working with medication administration systems and health-related documentation.
  • Experience working collaboratively with AMAP-certified personnel (preferred).

Scope of Practice

The Licensed Practical Nurse functions under the supervision and direction of the Director of Clinical Services, Registered Nurse, physician, or other authorized healthcare provider. The LPN role is intended to support implementation and monitoring of health-related services and does not independently perform comprehensive nursing assessments, independent nursing diagnoses, independent clinical determinations, or functions reserved under West Virginia law for Registered Nurses or licensed independent practitioners. The LPN is expected to escalate concerns requiring advanced clinical judgment, medication determinations, delegation authority, or assessment of medical stability to appropriate nursing or medical leadership.
