Job summary

Burlington Health & Rehabilitation Center in Burlington, Vermont, is hiring a dedicated Licensed Practical Nurse (LPN) to provide direct nursing care to residents. The LPN will supervise certified nursing assistants and ensure compliance with federal, state, and local regulations while supporting residents' needs.

You will work as part of an interdisciplinary team, administer medications, monitor residents, and contribute to care planning and quality care delivery in a long-term care setting.

Qualifications

  • A Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program.
  • Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the practicing state.
  • Current CPR certification.
  • Long-term care experience preferred but not required.

Responsibilities

  • Directs the daily activities of certified nursing assistants in accordance with current regulations and guidelines.
  • Ensures policies and procedures are followed by nursing personnel.
  • Participates in admission, discharge, and transfer of residents as required.
  • Observes changes in residents' status and notifies the physician and family, documenting accordingly.
  • Transcribes physician orders to medical records and carries out orders as written.
  • Prepares and administers medications as per physician orders and monitors for adverse effects.
  • Performs wound care as ordered and documents changes.
  • Schedules follow-up appointments and transportation needs as indicated.
  • Provides nursing leadership to the unit and collaborates with the interdisciplinary team.
  • Initiates and updates care plans; participates in survey processes.

  • Provides direct care skills such as colostomy changes, tube feedings, wound care, suctioning, IV administration, etc. in accordance with current policies and procedures
  • Performs rounds to ensure resident needs are being met and personnel are performing their assigned duties
  • Collaborates with other members of the interdisciplinary team as needed to ensure residents' needs are holistically met
  • Informs appropriate departments of concerns or complaints and files grievances as necessary
  • Reports any incidents or unusual occurrences to the supervisor, unit manager, assistant director, or director of nursing and participates in the investigative process as needed
  • Participates in end-of-life care as required
  • Ensures that there is adequate stock of medications, supplies, equipment, and notifies appropriate personnel of needs
  • Initiates, reviews, and updates care plans as required
  • Promotes teamwork, mutual respect, and effective communication
  • Participates in the survey process when inspection surveys arise
Required Qualifications
  • A Nursing Degree from an accredited college or university or a graduate of an approved LPN/LVN program
  • Prior experience in long-term care preferred but not required
  • Current unrestricted license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the practicing state
  • Current CPR certification
