Interim HealthCare Inc. is seeking an LPN for home care in Mann's Choice (Bedford). Schedules include 5AM–3PM or 10PM–10AM, with full-time, part-time, or PRN options.

You will provide in-home patient care, monitor progress, and support patients with dignity and comfort. Qualifications include a graduate of an accredited nursing program, an unrestricted LPN license in the state, CPR/AED/BLS/First Aid certification, and reliable transportation.

Qualifications

• Graduate of an accredited nursing program.
• Current unrestricted license to practice as LPN in the state for this position.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to/from care sites and/or work locations.

Responsibilities

• Provide in-home direct patient care according to the plan of care and nursing scope of practice.
• Assess and document patient conditions; report changes as needed (vital signs, urine, etc.).
• Administer medications, insulin, and IV fluids; document thoroughly.
• Inspect and care for wounds, change dressings, assist with hygiene.
• Assess for further injuries or infections and deliver appropriate treatments.
• Collaborate with providers and health professionals to ensure patient outcomes.

As a Licensed Practical Nurses (LPN) you will:

• Providein-homedirect patient care according to theprovider'sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Educational: Graduate of an accredited Nursing program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locationsPractical trachand/or ventilator experience preferred, not required
