Job summary

Elegance Senior Living is seeking a licensed caregiver to support residents according to each care plan and document changes per local rules and HIPAA guidelines.

You'll collaborate with the Director of Health and Wellness, participate in hands-on care, review care plans for accuracy, and provide clinical support and training to the direct care team. This role emphasizes safety, teamwork, and adherence to regulations across the community.

Responsibilities

  • Follows resident service plans and documents any condition changes per local rules and policies.
  • Communicates with the Director of Health and Wellness as needed to ensure quality services.
  • Provides hands-on care and wellness tasks aligned with each resident's care plan and state regulations.
  • Delivers clinical support and trains direct care staff to meet continuing education requirements.
  • Assists transcription of physicians' orders and reviews new orders for accuracy.
  • Maintains required certifications and completes ongoing in-service trainings and coursework.
  • Supports QA processes and reports issues to the Executive Director.
  • Reviews MARs and Daily Reports to ensure appropriate clinical interventions for residents on alert or with acuity changes.
  • Complies with HIPAA, OSHA, Bloodborne Pathogen guidelines and all applicable local/state/federal rules.
  • Promotes safety, assists with investigations and return-to-work processes after injuries, and supports management communication.
  • Demonstrates teamwork, initiative, and professional behavior with residents, families, visitors, physicians, and staff.
  • Performs other duties as assigned.

Job description

Responsibilities
  • Follows the services outlined in each resident's service plan in a friendly and supportive manner. Documents any observed change of condition in accordance with local rules and regulations and policies.
  • Communicates with Director of Health and Wellness on resident needs on an as needed basis to ensure consistent quality services.
  • Participates in providing hands on care and specific wellness tasks as required and in support of the resident's care plan. Reviews care plans and ensures the plan meets with resident's specific level of support as required by state regulations.
  • Provides clinical support and training of direct care team in support of continuing education requirements.
  • Assists in transcription of physicians' orders and the review of new physicians' orders.
  • Maintains the proper certifications required by local and state licensing agencies which include ongoing in-service trainings, accreditations, specific coursework and certifications.
  • Assists the community with compliance with the community quality assurance. Reports identified issues not supporting quality services to Executive Director.
  • Provides specific review of Medication Administration Records (MARs) and review of the Daily Report to ensure appropriate clinical interventions are implemented and reviewed on those residents on alert or with acuity changes.
  • Follows all local, state and federal rules and regulations as they apply to services delivered for all residents. This involves resident care and documentation and compliance with HIPAA, OSHA, and Blood Borne Pathogen Guidelines.
  • Works in a safe manner, supports other employees in working in a safe manner, and corrects unsafe actions. Compliant with Safety Committee Standards. Should workplace injuries occur, when required, assists the Executive Director and Director of Health and Wellness with the investigation, return to work, and management of the injured worker communication with care provider and Management Company.
  • Fosters teamwork with co-workers, shows initiative in overcoming obstacles, takes actions which assists others beyond job responsibilities, and has a positive impact on co-workers, residents, and visitors. Models professional behavior in communication with residents, families, visitors, physicians, and both clinical and non-clinical staff in the community.
