Job summary

Ambercare Hospice in Albuquerque, NM is seeking a compassionate LPN to join our team in providing direct patient care in a home hospice setting. You will perform assessments, implement the plan of care, document services accurately, and educate patients and families to support comfort and dignity.

This role offers a Monday–Friday daytime schedule with autonomy to coordinate visits, competitive pay plus mileage, and a comprehensive benefits package including medical, dental, vision, a 401(k)

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state.
• Prior home health or hospice experience preferred with minimum of 1 year medical/surgical experience.
• Maintains a current CPR certification.

Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.

What You’ll Do:

• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, Dietitian or Clinical Supervisor
• Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state.
• Prior home health or hospice experience preferred w/ minimum of 1 year medical/surgical experience.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
