Job summary

Community Options, Inc. seeks an experienced Licensed Professional Counselor (LPC) in Hamilton, NJ to develop and monitor individualized behavior plans for residents in group homes. You will collaborate with a multidisciplinary team to ensure high-quality, client-centered care and crisis response when needed.

The role requires a Master’s degree in counseling or related field and an active LPC license. Minimum one year of experience in residential settings, with strong assessment and intervention

Qualifications

• Master’s degree in counseling, psychology, social work, or a related field.
• Current and valid LPC license in the relevant state.
• Minimum of one year of experience working with adults in group homes or similar settings.
• Experience with behavior assessment, treatment planning, and implementing behavior interventions.
• Knowledge of mental health and developmental disabilities common in residential care.
• Strong knowledge of behavioral therapy and evidence-based intervention strategies.

Responsibilities

• Create individualized behavior plans for adults in group homes, tailored to address their needs and ensure measurable goals.
• Conduct behavioral assessments and functional analyses to identify triggers and underlying causes.
• Evaluate progress and adjust behavior plans as needed.
• Collaborate with group home staff, case managers, families, and other health professionals to implement plans.
• Provide training and guidance to staff on implementing strategies in behavior plans.
• Provide crisis intervention and ensure safety for residents and staff.
• Document progress and update records reflecting changes in behavior or conditions.
• Maintain compliance with legal, regulatory, and organizational standards.
