Job summary

Sweetwater Care in Fowler, CA is seeking an LVN supervisor to deliver nursing care within scope of practice and ensure patient needs are met in accordance with center policies and regulatory guidelines.

The role emphasizes infection control, quality improvement, and supervision of care delivery including medications and treatments. Experience in long-term care and familiarity with PointClickCare are preferred.

Qualifications

• Licensed LVN in California.
• One-year nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
• Long-term care experience required.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in the Quality Improvement Program.
• Uses the Quality Life Manual for patient identification, placement and monitoring.
• Notifies departments of patient changes (rooms, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Adminsisters treatments timely and per facility policy.
• Receives, transcribes and carries out physician orders as allowed by the Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California.
• One-year prior nursing management experience preferred.
• Experience in SNF/LTC settings preferred.
