Job summary

Goldwater Care is seeking a Social Services Director to plan, organize, implement, and evaluate the facility's Social Services Department in accordance with regulations and policies. The role ensures the psychosocial needs of residents are met on an individual basis and supports resident rights and well-being.

The director will lead an interdisciplinary team, develop care plans, and coordinate discharge planning while educating staff and families about services and resources.

Qualifications

• Bachelor's Degree in relevant field is preferred.
• A minimum of two years experience in social services.
• Long term care experience is preferred.

Responsibilities

• Review facility policies and procedures to assure compliance with state and federal regulations.
• Develop and implement social work policies and plans of care.
• Complete a social history and psychosocial assessment for each resident.
• Provide therapeutic interventions to assist residents in coping with transition and adjustment to a long-term care facility.
• Coordinate the resident discharge planning process and make referrals for home care services.
• Educate staff and families about residents' rights and resources.
