Fundamental in Las Vegas is seeking a Social Services Assistant for a full-time role. The position offers an hourly wage range of $18.00–$20.00 and requires a high school diploma or GED with experience in long-term care or skilled nursing facilities.

Reliable transportation is essential. The role includes initial assessments, patient interviews for discharge planning, and coordination with Case Management to support patient discharges and access to Medicare/Medicaid resources.

Qualifications

• Must have a High School Diploma or equivalent (G.E.D).
• Social Work license preferred, not required.
• Experience in long term care or skilled nursing facility.
• Reliable transportation required.

Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission.
• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident.
• Work closely with Case Management to coordinate patient discharges.
