MedCap Health in Michigan is seeking a driven Long Term Care Physician – Post-Acute Care Doctor for a full-time role. Lead care teams in skilled nursing facilities and manage complex post-acute cases discharged from hospitals.

Collaborate with Nurse Practitioners and specialists to deliver hospital-level care in long-term settings, ensuring continuity, quality, and compassionate patient interactions. A flexible schedule and supportive environment are offered to accommodate work-life balance.

Competenze

• Board certification or eligibility in internal medicine, geriatrics, critical care, or family practice.
• Hospitalist experience or prior leadership roles preferred.
• Experience leading care teams in skilled nursing facilities is a plus.

WHAT WE OFFER

Pay: $100,000-$250,000/year

POSITION DETAILS:

• Manage complex cases discharged early from hospitals

What You Need:

• Board certification or board-eligible internal medicine, geriatrics, critical care, or family practice

Previous experience as a hospitalist or in leadership positions is strongly preferred.
