Job summary

MedCap Health seeks a driven full-time Long Term Care Physician – Post-Acute Care Doctor to lead clinical teams in our growing post-acute care network. You will oversee hospital-to-long-term care transitions, manage complex cases, and collaborate with NPs and specialists to deliver hospital-level care in a long-term setting while maintaining high-quality, compassionate patient care.

• Board certification or board-eligible in internal medicine, geriatrics, critical care, or family practice.
• Hospitalist or leadership experience preferred.
• Experience leading care teams in long-term/post-acute care settings.

POSITION DETAILS:

• Lead care teams in skilled nursing facilities
• Manage complex cases discharged early from hospitals
• Collaborate with Nurse Practitioners and specialists to deliver hospital-level care in a long-term setting
• Ensure continuity, quality, and compassion in every patient interaction

What You Need:

• Board certification or board-eligible internal medicine, geriatrics, critical care, or family practice

Previous experience as a hospitalist or in leadership positions is strongly preferred.
