Job summary

Sweetwater Care in Los Angeles is seeking a licensed LVN/LPN to provide nursing care within the scope of practice, following physician orders and center policies in line with applicable state, federal, and local guidelines.

The role requires documenting patient conditions, adhering to charting procedures, narcotic counting, and coordinating with physicians. Long-term care experience and BLS/CPR certification are valued; nursing management experience is a plus though not required.

Qualifications

• Experience in long-term care settings.
• Proficient with paper charting and Point Click Care charting systems.
• Licensed LVN/LPN in the practicing state.
• Current BLS/CPR certification or willingness to obtain one.
• Nursing management experience is preferred but not required.

Responsibilities

• Follow established procedures for charting and incident reporting.
• Follow procedures for narcotic counting, pharmacy orders, and restraint reduction.
• Document changes in patient conditions including nutrition, infections, skin issues, weight loss or other changes.
• Use the Quality Life Manual for patient placement and monitoring in appropriate programs.
• Document and notify applicable departments of patient room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.
