Job summary

Cherry Hill Free Clinic is seeking a Long-Term Care Functional Screener to build trusting relationships with IRIS participants and administer LTC Functional Screens. The role includes documenting renewals, guiding funding eligibility, and collaborating with team members and partners to strengthen community connections.

You will participate in meetings, prepare reports, testify in hearings, and ensure documentation standards are met while promoting participant autonomy and meaningful living.

Qualifications

• Bachelor's degree in social work, psychology, human services, nursing, or related field.
• Minimum 1 year of experience with seniors or individuals with disabilities.
• LTC FS certification or willingness to obtain.
• Bilingual skills a plus.

Responsibilities:

Build trusting relationships with IRIS participants, administer and coordinate LTC Functional Screens, document renewals, provide guidance on maintaining funding eligibility, collaborate with team members and community partners, participate in meetings and conferences, prepare reports, testify in hearings, ensure compliance with documentation standards, and strengthen community connections.

Qualifications & Skills:

Bachelor's degree in social work, psychology, human services, nursing, or related field; minimum 1 year of experience with seniors or individuals with disabilities; valid LTC FS certification or willingness to obtain; bilingual skills a plus; strong organizational, planning, and communication skills; proficiency with smartphones, tablets, and computers; commitment to participant autonomy and meaningful living.

Key Requirements:

Valid driver's license, reliable vehicle, satisfactory driving record, pass background check, and willingness to travel frequently including overnight trips.
