Job summary

MedCap Health in Stephenson, MI is seeking a full-time LongTerm and Post-Acute Care APP to join a physician-led team. You will manage patients with acute and chronic diseases, collaborate with the attending physician, and contribute to high-quality care in post-acute settings.

This role offers a day schedule with no overnights or weekend shifts, and autonomy supported by a multidisciplinary team including Nursing, Social Work, and Administrative staff, plus malpractice coverage.

Qualifications

• Board-certified Nurse Practitioner required (geriatric, family, or adult).
• 1+ years of post-acute/SNF/LTC experience preferred.
• Post-acute/long-term/post-acute facility experience strongly preferred.

Responsibilities

• Manage patients with acute and chronic diseases as part of a multidisciplinary team.
• Collaborate directly with the attending physician to deliver high-quality care.
• Coordinate with Director of Nursing, Social Workers, and other staff to ensure cohesive care.

YOUR ROLE AS A LONG TERM APP:

Your schedule is Monday through Friday, day shifts, with no nights or weekends required.

As a key member of a multidisciplinary team, you play a central role in patient care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.

OUR IDEAL PRACTITIONER

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
