Job summary

MedCap Health is seeking a full-time/part-time, flexible schedule Long Term and Post-Acute Care APP to reshape the future of post-acute care. You will join a multidisciplinary team, manage patients with acute and chronic diseases, and collaborate with physicians to deliver high-quality care.

Ideal candidates are board-certified NPs (geriatric, family, or adult) with 1+ years in post-acute/SNF/LTC, and prior SNF/long-term care experience.

Qualifications

• Board-certified NP with geriatric, family, or adult focus.
• 1+ year of post-acute/SNF/LTC experience.
• Prior post-acute, long-term, and/or skilled nursing facility exposure.

Responsibilities

• Manage patients with acute and chronic diseases in collaboration with the attending physician.
• Coordinate with Director of Nursing, Social Workers, and the care team to ensure comprehensive care.
• Provide high-quality, autonomous patient care with schedule flexibility.

YOUR ROLE AS A LONG TERM APP:

As a key member of a multidisciplinary team, you play a central role in patient care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.

OUR IDEAL PRACTITIONER

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
