MedCap Health is seeking a full-time LongTerm and Post-Acute Care APP to join a physician-led organization focused on reshaping post-acute care. Enjoy a Monday–Friday day shift with no overnights or weekends, malpractice coverage, and administrative support.

As a key member of a multidisciplinary team, you will manage patients with acute and chronic diseases, collaborating with physicians and nurses to provide top-quality care and improve patient outcomes.

Qualificações

• Board-certified NP in geriatrics, family, or adult care.
• 1+ years of experience in post-acute/SNF/LTC settings.
• Experience in post-acute, long-term, and/or skilled nursing facilities.

Responsabilidades

• Manage patients with acute and chronic diseases on a day shift.
• Collaborate with attending physician to deliver high-quality care.
• Coordinate with Director of Nursing and social workers for comprehensive care.

YOUR ROLE AS A LONG TERM APP:

Your schedule is Monday through Friday, day shifts, with no nights or weekends required.

As a key member of a multidisciplinary team, you play a central role in patient care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.

OUR IDEAL PRACTITIONER

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
