MedCap Health is seeking a full-time LongTerm and Post-Acute Care APP (NP/PA) to reshape the future of post-acute care. Your schedule will be Monday through Friday, day shifts, with no nights or weekends required.

As a key member of a multidisciplinary team, you will manage patients with acute and chronic diseases, collaborate with the attending physician, and work closely with the Director of Nursing and Social Workers to ensure comprehensive, high-quality care and improved quality of life.

Formación

• Board-certified NP in geriatrics/family/adult
• 1+ year post-acute/SNF/LTC experience
• Experience in long-term/skilled nursing facilities

Responsabilidades

• Manage patients with acute and chronic diseases.
• Collaborate with attending physician to provide high-quality care.
• Communicate with Director of Nursing, Social Workers, and team members.
• Help improve patients' quality of life through coordinated care.

YOUR ROLE AS A LONG TERM APP:

Your schedule is Monday through Friday, day shifts, with no nights or weekends required.

As a key member of a multidisciplinary team, you play a central role in patient care. You'll spend your days managing patients with acute and chronic diseases, collaborating directly with the attending physician to provide the highest quality of care. Your expertise is crucial in effectively managing patients with debilitating chronic diseases, improving their quality of life. You are a natural collaborator, communicating and working seamlessly with the Director of Nursing, Social Workers, and other team members to ensure a cohesive and comprehensive approach to care. This dynamic role keeps you engaged and connected as you make a tangible difference in the lives of those you serve.

OUR IDEAL PRACTITIONER

• A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• 1+ years of experience in post-acute/SNF/LTC
• Prior post-acute, long-term, and/or skilled nursing facility experience
