Post Acute Care NP opening in Wilkes-Barre

Location:

Wilkes-Barre, PA

Position:

Full-time, permanent

Certification:

Board Certified

Essential Job Functions:
  1. Provide medical services for Skilled Nursing, Rehabilitation, Long Term Care, and Assisted Living facilities.
  2. Perform exams, assessments, and develop treatment plans competently.
  3. Follow policies regarding the frequency of care oversight and note reviews for patients in skilled nursing facilities.
  4. Facilitate patient transitions within and between healthcare settings, including admissions, transfers, and discharges.
  5. Collaborate with multidisciplinary team members.
  6. Submit accurate and timely charges for all professional services.
Requirements:
  • At least 1 year of experience as an Advanced Practice Provider in an acute role, preferably in long-term care, skilled nursing, or similar inpatient settings.
  • Graduate of an accredited school and program relevant to the NP role.
  • Licensure to practice medicine in good standing with the state governing body.
  • Federal DEA certification in good standing.
  • State CDS in good standing.
  • Ability to be credentialed at designated locations.
  • Strong clinical skills with competency in evaluation, diagnosis, and treatment.
