Essential Job Functions:
- Provide medical services for Skilled Nursing, Rehabilitation, Long Term Care, and Assisted Living facilities.
- Perform exams, assessments, and develop treatment plans competently.
- Follow policies regarding the frequency of care oversight and note reviews for patients in skilled nursing facilities.
- Facilitate patient transitions within and between healthcare settings, including admissions, transfers, and discharges.
- Collaborate with multidisciplinary team members.
- Submit accurate and timely charges for all professional services.
Requirements:
- At least 1 year of experience as an Advanced Practice Provider in an acute role, preferably in long-term care, skilled nursing, or similar inpatient settings.
- Graduate of an accredited school and program relevant to the NP role.
- Licensure to practice medicine in good standing with the state governing body.
- Federal DEA certification in good standing.
- State CDS in good standing.
- Ability to be credentialed at designated locations.
- Strong clinical skills with competency in evaluation, diagnosis, and treatment.
