Locums Nurse Practitioner or Physician Assistant – Home Care Visits

We are seeking a Locums Nurse Practitioner (NP) or Physician Assistant (PA) for a flexible position split between Amityville and Flushing, NY. The role involves providing comprehensive primary care to geriatric PACE participants in their homes, ensuring high-quality, patient-centered care.

1. Small daily patient census: 3-5 patients per day
2. Flexible schedule: 4 days per week, with two days in Amityville and two days in Flushing
3. No nights, no weekends

Compensation: $75-80 per hour

Requirements:

• Active NP or PA Board Certification
• Medicare & Medicaid Provider ID (REQUIRED)
• Minimum of 2 years of experience preferred
• Spanish speaking is preferred, but not required
• Experience in home care and geriatrics preferred

Both Amityville and Flushing are located in New York, offering different experiences and settings.
