Qualifications

Responsibilities

• Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to geriatric patients.
• Diagnose, treat, prescribe medications, and develop individualized treatment plans.
• Order and interpret diagnostic tests (labs, imaging, etc.).
• Document accurately in the EHR system.
• Provide patient education and promote wellness initiatives.
• Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care.
• Refer patients to specialists and manage follow-ups.
• Ensure compliance with state, federal, and professional regulations.
