Job Description & Requirements

Position Type: Temporary or Temp-to-Perm (the client will consider two temporary providers)

Certifications Required:

• Board Certified through ANCC or AANP
• DEA (pending DEA accepted up to 60 days)
• BLS (hands-on preferred, willing to take a course if not)

Licensure: Indiana State Licensure

Experience: Minimum 1-2 years of NP experience (urgent need if less)

Start Date: ASAP (within 2-4 weeks after potential offer to credential, ideally by early June)

Duration: Until July 31, 2025, with potential to extend or transition to permanent

Schedule:

• 4x10-hour shifts: Mon/Thurs/Fri 7am-6pm, Tues 8am-7pm
• Saturday rotation every 8 weeks (8am-12pm)

On-call / Callback: None

Compensation: $70 - $80 per hour (negotiable)

Responsibilities:

• Provide care at an Employee Health and Wellness Clinic for employees and their dependents
• Treat patients aged 2+ for acute and chronic conditions
• Conduct well-woman exams and minor procedures
• Collaborate with a multidisciplinary team including behavioral health, massage therapist, psych NP, dietician, chronic care management counselors, and physical therapy

Additional Information:

• Patient Volume: 16-18 patients per day on average
• EMR System: EPIC
• Support Staff: 1-2 MA or LVN per provider, 4 physicians on site, 2-3 other NPs
• Clinic: New facility built in 2015

Interview Process:

• Ideally in person with the Medical Director and the Health Center Manager
• If not in person, then via Teams or Zoom

Onboarding:

• Orientation and onboarding: 2-4 weeks
