Job summary

DocStar Medical Partners in Vacaville, CA is seeking a Family Nurse Practitioner (FNP) for a locum-to-perm coverage at a FQHC. You will provide primary care across all ages, including preventive health, immunizations, and chronic disease management, using EPIC EMR.

Schedule Monday–Friday 8:00 AM–5:00 PM; average 18–20 patients daily; CA license and BC certification required; 2-3 weeks credentialing; OT not paid unless approved; 5-day work week; commitment to coverage.

Qualifications

• FQHC experience is required.
• EPIC EMR experience is required.
• CA license to practice is required.
• Board Certification in Family Medicine is required.

Responsibilities

• Provide Family Medicine and primary care for all ages.
• Perform preventive health visits and immunizations.
• Manage chronic diseases and medication plans.
• Coordinate care and document in EPIC EMR.
