- Reason for Coverage:Current locum unable to fill these dates.
- Deal breakers: Candidate must be able to cover full Feb stint, no partial shifts will be accepted.
Submission Requirements

- Active CO license -Required or IMLC (LOQ in hand at time of submission)
- Active BLS-Required for credentialing
- Active DEA -Required for credentialing (site will consider a transferred DEA, however, must keep this DEA for the duration of the assignment, not just for shifts worked.)
- Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date-Required
- Candidate must be available forany of the stints in job description to be considered-Required
- Clean MP/Background-Highly Preferred, candidates who present with a self inquiry NPDB report will be placed in priority over those that do not include this.
