**Schedule Overview:**

• **Dates:** - December 14 – 18


• - December 21 – 28 (weekend on-call until 07:00 on December 28)


• - December 28 – 31 (Note: No assignments on New Year's Day) -

**Shift Details:**

- **Hours:** 07:00 to 15:00 - **Duration:** 8-hour shift (no option for 10-hour shifts)

**Candidate Requirements:**

• Must be board certified


• Hold valid IN License/IMLC, CSR, and DEA registration


• Must be a drive-in candidate - Commitment to all specified dates is essential


• Required to sign Anesthesia Attestation for all procedures


• Fully credentialed with their locum agency


• Must commit to orientation on Friday, December 11, 2026 - Case logs will be necessary
