Inpatient Pharmacists (Hospital)

Position Overview

Clinical Pharmacist (Anticoag, Chronic Disease, Transitions of Care)

Location: California

Work Type: Onsite Locum Tenens

Compensation: Competitive hourly rates (role-dependent)

Classification: 1099 Independent Contractor

Schedule: Flexible (Per-Diem, Part-Time, Full-Time, Contract)

Responsibilities

• Verify and process medication orders with accuracy.
• Prepare, dispense, and oversee medication distribution.
• Conduct patient medication counseling and education.
• Support clinical programs such as:
  • Anticoagulation
  • Antibiotic stewardship
  • Diabetes & chronic disease therapy
  • Medication therapy management (MTM)
• Collaborate with physicians, nurses, and care teams.
• Document all activities in the EMR accurately and timely.
• Ensure compliance with state and federal regulations.
• Oversee day-to-day pharmacy operations.
• Supervise pharmacists, technicians, and support staff.
• Manage workflow, staffing schedules, and operational efficiency.
• Ensure compliance with Joint Commission, CMS, and California Board of Pharmacy standards.
• Lead clinical initiatives, policy implementation, and quality improvement projects.
• Participate in hiring, onboarding, and performance evaluations.
• Serve as the primary operational point of contact for leadership.

Requirements (Qualifications)

• Active California Pharmacist License (RPh or PharmD).

• 2+ years of leadership experience preferred.
• Strong understanding of compliance & regulatory standards.
