Job summary

Community Behavioral Nursing Services in Amherst, Ohio seeks a dependable LPN or RN for temporary full-time home health assignments across Stark, Medina, Cuyahoga, and Lorain counties. You will provide skilled nursing care per the current plan of care, document vital signs, coordinate care with CBNS staff and other providers, and promote safety and patient education.

The position offers a bi-weekly salary of $2,971.02–$3,517.17, with scheduling on Tue/Thu mornings and evenings, plus alternate

Qualifications

• Active, unencumbered Ohio LPN or RN license.
• Minimum 1 year of LPN/RN experience, preferably home care.
• CPR certification level current.
• Valid driver's license and auto insurance.

Responsibilities

• Provide skilled nursing care per plan of care within scope of practice.
• Document vital signs, observations, and body systems.
• Complete skilled nursing procedures and specimen collection.
• Educate patients and caregivers to reduce hospitalizations.
• Coordinate care with CBNS staff and other providers.
• Process medication-related tasks and orders coordination.
• Maintain safe environment and escalate issues when needed.
• Complete shift reports and incident/infection documentation.
