IDR Healthcare is seeking a local contract nurse RN Case Management for a local contract nursing job in Dallas, Texas.

Job Description & Requirements

100% onsite M-F 8a -4:30pm (No weekends on call).

We are seeking a dedicated and team-oriented Inpatient Case Manager to join a Case Management department. This is an opportunity ideal for someone who thrives in a collaborative hospital setting and is passionate about ensuring high-quality patient outcomes.

Key Responsibilities

• Manage care coordination and discharge planning for inpatient populations.
• Maintain a patient caseload of approximately 18–25 patients.
• Collaborate with physicians, nurses, social workers, and external partners to ensure seamless transitions of care.
• Document effectively in Cerner (EMR system).
• Participate in interdisciplinary rounds and planning meetings.
  
  

Must-Have Qualifications

• 1-3 years of Case Management Experience
• Certifications: BLS
