Qualifications

  • Valid driver’s license and reliable transportation for patient visits
Roles & Responsibilities
  • Provide skilled nursing care based on physician-prescribed plans of care
  • Administer medications, treatments, and therapies with precision and compassion
  • Document all nursing assessments, actions, patient responses, and care coordination accurately
  • Educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Monitor patient conditions and communicate changes to the interdisciplinary team
  • Deliver 1:1 patient care with a focus on medically fragile individuals
Requirements
  • Flexibility to work full-time or part-time schedules
  • Ability to manage individualized caseloads in home settings
  • Willingness to participate in ongoing training and professional development
  • Service coverage in the following counties: Skagit, King, Snohomish, Kitsap, Pierce, Thurston, Grays Harbor
