Job Title: RN Case Manager – Medical/Surgical & Telemetry
Shift: 10-hour AM shifts, 4 days/week (40 hours total)
Schedule: Rotating days, no fixed day off; every 6th weekend; no major holidays
Location: Boston, MA

Position Summary:

We are seeking a highly experienced Registered Nurse Case Manager to support the Medical/Surgical and Telemetry unit (Menino 7 West). This role is integral in coordinating care for patients with complex needs, ensuring appropriate resource utilization, and achieving quality, cost-effective outcomes. The RN Case Manager will work with a panel of 18–22 patients using a collaborative, interdisciplinary approach to promote efficient patient care planning and discharge processes.

Key Responsibilities:

• Assess, plan, implement, monitor, and evaluate care plans and discharge needs for acute med/surg and stroke patients
• Utilize clinical knowledge to determine medical necessity using InterQual criteria and inpatient Medicare lists
• Coordinate services with the interdisciplinary team to meet patient health needs and reduce length of stay
• Communicate effectively with patients, families, providers, payers, and internal teams
• Apply knowledge of health plans, benefits, eligibility, and utilization management criteria
• Document all activities clearly and efficiently using EPIC

Required Qualifications:

• Bachelor of Science in Nursing (BSN)
• Minimum 3 years of recent experience in Med/Surg with strong inpatient care knowledge
• In-depth understanding of physical/mental impairment assessments, illness characteristics, and treatment expectations
• Familiarity with benefits, insurance plan contracts, and eligibility issues
• Strong clinical judgment and critical thinking

Preferred Qualifications:

• 5+ years in Case Management and Utilization Management
• Experience using EPIC
• Certification in Case Management (CCM or related)

Department Overview:

• Unit: Medical/Surgical & Telemetry (Menino 7 West)
• Patient Population: Stroke and Med/Surg patients
• Patient Load: 18–22 patients
• Primary Goals: Manage LOS, ensure appropriate utilization, and facilitate smooth discharge planning
